Wellness Center Patient Rights & Responsibilities
As a recipient of Wellness Center services, you have a right to:
Considerate and respectful care, including respect for your cultural, psychosocial, spiritual, and personal values, beliefs, and preferences. You have the right to receive care in a safe setting, free from all forms of abuse and neglect, exploitation, harassment, or discrimination.
Respect for your personal privacy and confidential treatment of all communications and records pertaining to you. You will receive separate notice or other documents (such as a “Notice of Privacy Practices” in the US) explaining your privacy rights in detail and how we may use and disclose your protected health information. You also have the right to obtain copies or summaries of your medical records consistent with applicable laws.
Receive information and ask questions about your health status, diagnosis, prognosis, recommended course of treatment, and outcomes of care, make decisions regarding your care, and participate in developing and implementing your plan of care. This includes the right to reasonable responses to any reasonable requests for services as determined by your care providers. You do not have the right to demand inappropriate or unnecessary treatment or services.
Be advised of any conflicts of interest your provider may have with respect to your care and to be advised if your providers propose to engage in or perform research affecting your care or treatment. You may refuse to participate in any research.
Reasonable continuity of care and appropriate coordination of care between your providers. Your provider will not discontinue treating you when further treatment is indicated without giving you sufficient notice and reasonable assistance in making alternative arrangements.
As a recipient of Wellness Center services, you have the responsibility to:
Provide, to the best of your knowledge, accurate and complete information about all matters relating to your health and any medications. Tell your provider about any unexpected changes in your health, and accept the consequences if you choose to refuse treatment or do not follow the instructions your provider gives to you.
Recognize that a healthy lifestyle can often prevent or mitigate illness and take responsibility for your health by following preventive measures and adopting health-enhancing behaviors.
Keep scheduled appointments and, when unable to do so for any reason, notify the Wellness Center as soon as possible before your appointment time and in accordance with the Wellness Center’s cancellation and no-show policy.
Follow Wellness Center rules and policies affecting patient care and refrain from behavior that unreasonably places the health of others at risk. Do what you can to help prevent transmission of infectious disease.
Respect the rights and property of providers, staff, and other patients at the Wellness Center, including by refraining from inappropriate, discriminatory, harassing, abusive, or disruptive language and behavior. Comply with all applicable Apple policies, such as Apple’s Business Conduct Policy, if you are an Apple employee. Be aware of the Wellness Center’s Zero Tolerance Policy below. Do not damage or destroy Wellness Center equipment or property.
Maintain professional boundaries in the patient-provider relationship, which serve to protect both you and your provider. Your provider shares this responsibility.
Timely meet your financial responsibilities, including by paying any applicable fees or co-payments for services at the time of your appointment.
Zero Tolerance Policy
The Apple Wellness Center has a zero tolerance policy. Any sexual advances, innuendo, inappropriate touching, displays of aggression or undraping without permission will not be tolerated, and is grounds for immediate termination of your session. Depending on the nature of the circumstances, such acts may also lead to a cessation of any future services to you.
Apple Wellness is dedicated to providing you with the best possible care, and we want to be sure you are treated with dignity and respect. As a recipient of care, you are also expected to treat all members of our staff the same way.
