Health Partner
As an integral member of the primary care team, the Health Partner works in direct partnership with the primary care physician to help patients improve their health and wellness through sustainable behavior change.
Health Partner (Behavioral Health)
As an integral member of the primary care team, the Behavioral Health Partner works in direct partnership with the primary care physician to help patients improve health and wellness through sustainable behavior change.
Clinical Exercise Coach
The Clinical Exercise Coach works in direct partnership with the primary care team to help patients improve health and wellness through sustainable exercise and physical activity-related behavior change.
Primary Care Physician
An AC Wellness physician approaches the patient in a holistic fashion, with emphasis on preventing future disease but with a confident approach to acute and chronic existing disease.
Acute Care Physician
This role provides primary health care, episodic, and preventive care to patients. An emphasis on patient-centered care with a high-touch/concierge-like approach is essential to how care is delivered.
Care Navigator
The care navigator sets the tone for the patient experience, whether it is the patient’s first phone call, or their first interaction with a team member entering the center. The care navigator acts as a care navigator for patients in and out of the center.
Phlebotomist
Under the direct supervision of the Medical Director, the phlebotomist is responsible for phlebotomy, accessioning, receiving and specimen processing, report distribution. Assists in the training of new employees and/or students as needed.
Medical Assistant
The Medical Assistant works within the care team to create a frictionless experience for patients of the Wellness Center. The Medical Assistant assists the care team in coordinating patient care and the flow of patients throughout the clinic.
Nurse Coordinator
The Nurse Coordinator works to create a frictionless experience for all patients. The nurse performs all steps of the nursing process including assessing patients, interpreting data, planning, implementing, and evaluating care; coordinating care with other providers; and teaching the patient and family the knowledge and skills needed to manage their care to prevent complications
Nurse Practitioner
The Advanced Practice Provider practices as an independent practitioner to provide routine care and management of the patient with acute and chronic complaints, in collaboration with the supervising physician. This role provides primary health care, episodic, and preventive care to patients.
